East was squeezed in three suits. He can't give up another heart since you can set up a heart by leading low from dummy. And a club discard gives up a long club in dummy. The spade discard avoided the issue for a moment, but because East no longer has enough spades to hurt you, it is possible to throw him with his spade winner. A curious hand.
